Penicillin-resistant pneumococci play only a minor role in antibiotic therapy. Occasionally occurring intermediate pencillin-sensitive pneumococci can be treated well with higher doses of pencillin or with a 3rd gene cephalosporin(ceftriaxone or cefotaxime). In rare pneumococcal infections with pronounced penicillin resistance (especially in the presence of meningitis), ceftriaxone should be combined with vancomycin. Alternatively, ceftarolin may be considered.
