Photopherese extrakorporale

PUVA-Therapie der Leukozyten in einem extrakorporalen Kreislauf (Leukapherese-basierte immunmodulatorische Therapie). Direkte zytotoxische/zytostatische Wirkung von Methoxsalen mittels gezielter Bestrahlung insbesondere der im Blut zirkulierenden Lymphozyten mit UVA (365 nm; sog. Photoadduktion des Methoxsalen an die DNA-Helix). Man vermutet, dass es durch die Reinfusion der mit Methoxsalen oberflächenmarkierten Lymphozyten zu einer Stimulation des Immunsystems gegen Antigendeterminanten der autoreaktiven bzw. malignen Zellklone kommt.

Die Behandlungsdauer einer extrakorporalen Photopherese (ECP) beträgt ca. 3-4 Std. Im extrakorporalen Kreislauf befinden sich 540 ml Leukozyten-reiches Blutplasma, kumulative GD UVA ca. 2 J/cm². Durchführung eines Zyklus an 2 aufeinander folgenden Tagen. Zyklusintervalle richten sich nach Erkrankung und Therapieerfolg (s. Tabelle 1).

Der Wirkmechanismus der ECP ist bisher noch nicht vollständig aufgeklärt. Die ECP gilt als immunmodulatorische Therapieform mit Verschiebung des Zytokinexpressionsmuster von einem proinflammatorischen zu einem antiinflammatorischen Profil. Die ECP bewirkt eine Zunahme der peripheren CD3/NK-Population und eine Minderung des Granulozyten-Elastase-alpha 1-Proteaseinhibitor-Komplexes sowie einen Abfall der Sulfidoleukotriene u. Stimulation der Synthese des TNF-α. Weiterhin nachgewiesen ist eine Apoptose mononukleärer Zellen, sowie eine Induktion von CD4/CD25-positiven T-regulatorischen Zellen.

Zugelassen ist die extrakorporale Photopherese bisher nur für das kutane T-Zell-Lymphom! S. Tabelle 1.

Insuffizienter peripherer Zugang, so dass die minimale Flussrate von 7 ml/Min. nicht gewährt ist. Hier können permanente, subkutan implantierbare Hämodialysekatheter mit einer Länge von max. 48 cm (Fa. Covidien Healthcare Deutschland [früher Tyco Healthcare, Deutschland]) ausreichende Flussvolumina garantieren.
