Definition

Seltene, pruritische Hautreaktion mit Histoeosinophilie, die im Verlauf einer Strahlentherapie am Bestrahlungsort auftreten kann.

Ätiopathogenese

Die Ätiopathogenese von EPPER ist unbekannt. Anzunehmen ist eine reaktive Strahlen-induzierte kutane Immunreaktion, die eine lokale Zytokinkaskade mit konsekutiver Eosinophilie auslöst. Bemerkenswert ist, dass eine strahleninduzierte, tumorassoziierte-Gewebeeosinophilie als ein gutes prognostisches Zeichen bei Gebärmutterhalskrebs beschrieben wurde, wobei postuliert wurde, dass die Gewebeeosinophilie durch die Bildung oder Transformation eines Antigens aus dem Tumor infolge der Strahlentherapie entsteht, die beim Wirt eine Dysregulation des Immunsystems auslöst (Rueda R et al.1999).

Klinisches Bild

Sowohl innerhalb als auch außerhalb des Bestrahlungsfeldes kommt es zu juckenden polymophen, sattroten Papeln (s.Abb.), die z.T. mehrere Wochen nach Bestrahlungsbeginn auftreten. Beschrieben wurde derartige „Reaktionen“  nach Bestrahlungen von Brust- und Mammakarzinomen. In einem Fall trat diese Reaktion nach Bestrahlung eines Merkelzell-Karzinoms auf.

Histologie

Dermales Ödem, dichtes dermales, perivaskulär orientiertes eosinophiles Infiltrat. Subepitheliale Blasenbildung möglich. Keine spezifischen Immunfluoreszenzphänomene.

Therapie

Glukokortikoidexterna.
