Definition

Äußerst seltener, intralymphatisch wachsender Gefäßtumor des Kindesalters unklarer Dignität.

Manifestation

V.a. im frühen Kindesalter auftretend; seltener kongenital; seltener bei älteren Patienten (Ø 30 Jahre).

Lokalisation

Kopf, Hals, Nacken, Rumpf; seltener Extremitäten.

Klinisches Bild

Dermatologisch uncharakteristisch. Bis 40 cm Ø große, feste, rote bis blau-rote knotige oder flach tuberierende Schwellungen der Haut und des subkutanen Fettgewebes. Häufig in präexistenten lymphatischen Malformationen oder in Lymphangiomen entstehend.

Histologie

In der Dermis oder Subkutis gelegene, aber auch interne Organsysteme befallend (z.B. Knochenstrukturen) intralymphatische Proliferate aus traubenförmigen Papillen (dabskoid tufts) mit homogenem bindegewebigem Grundstock (Immunreaktiv für Kollagen Typ IV) die mit Hobnail-Endothelien überzogen sind. Hieraus ergibt sich ein glomeruloider (nierenglomerulaähnlich) Aspekt. Fokale lymphoidzellige Infiltrate.

Immunhistologie: Glattmuskelaktin-positive Perizyten fehlen (spricht für lymphatische Genese); Endothelien exprimieren u.a. VEGFR-3 (Marker für lymphatische Endothelien) uind Podoplanin.

Differentialdiagnose

Verlauf/Prognose

Dignität zweifelhaft. Nach operativer Entfernung hohe Rezidivquote (40%); regionäre Lymphknotenmetastasen sind möglich aber selten, sehr selten Fernmetastasen.
