Definition

Interleukin-5 ist ein aus 115 Aminosäuren bestehendes Glykoprotein (Zytokin). Das kodierende Gen ist auf dem 5. Chromosom lokalisiert.

Eosinophile Granulozyten sind die Hauptquelle von Interleukin-5. Weiterhin wird das Zytokin von TH2-Helfer-Zellen und Mastzellen sezerniert und fungiert als spezifischer hämatopoetischer Wachstumsfaktor. IL-5 fördert Differenzierung von B-Zellen und weiterhin die Synthese von IgA durch Plasmazellen. Das Zytokin bewirkt die Transformation von Thymozyten in zytotoxische T-Lymphozyten.

Interleukin-5 ist mitbestimmend für die Regulierung von Proliferation und Differenzierung eosinophiler Granulozyten. Bei Überexpression von Il-5 kommt es zu Eosinophilien. 

Interleukin-5-Rezeptoren werden von allen hämatopoetischen (v.a. jedoch  von eosinophilen Zellen) exprimiert. Sie liegen als hochaffine und niedrigaffine Rezeptoren vor. Neben den membranständigen Rezeptoren liegen auch lösliche Rezeptoren (sIL-5R) vor. 

Allgemeine Information

IL-5 bewirkt die Aktivierung von eosinophilen Granulozyten, beeinflusst die generelle Immunantwort und entzündliche Prozesse. Es spielt eine Rolle bei der Entstehung von Allergien, der eosinophilen Ösophagitis, des Hypereosinophilie-Syndroms wie auch der hypereosinophilen Dermatitis.

 

Therapie allgemein

Anti-Interleukin-5-Therapien werden experimentell bei eosinophilen Erkrankungen durchgeführt. Einsatz von monoklonalen humanisierten Antikörpers gegen humanes IL-5 ( Mepolizumab und Reslizumab) beim Asthma bronchiale, beim atopischen Ekzem (keine signifikaten Ergebnisse), beim Hypereosinophilie-Syndrom sowie der Hypereosinophilen Dermatitis (deutliche Besserung) .

Eine UVA-Therapie der atopischen Dermatitis führte trotz Besserung des kisichens Status zu keiner Änderung der Interleukin-5 Expression.   

Monoklonale Antikörper gegen den Rezeptor "Interleukin-5Ralpha"(Benralizumab) werden bei Patienten mit eosinophilem Asthma bronchiale eingesetzt.
