Definition

Antibiotikum mit zytostatischer Einsatzmöglichkeit: Durch die Einlagerung in die Erbsubstanz kommt es zu einem Bruch der DNA-Stränge.

Pharmakodynamik (Wirkung)

Bleomycin induziert in experimentellen Systemen die vermehrte Expression extrazellulärer Matrixproteine, insbes. von Alpha(I)kollagen, Fibronectin und Decorin. Darüber hinaus stimuliert Bleomycin Zytokine, die in der dermalen Fibrose eine Rolle spielen, wie das "monocyte chemoattractant protein-1" (MCP-1 Protein) und einen seiner Rezeptoren (CCR-2).

Unerwünschte Wirkungen

Kutane UAWs: Pruritus, Toxische epidermale Nekrolyse, Angioödeme, streifenförmige und generalisierte Hyperpigmentierungen, Melanodermia factitia, flächige Hautsklerosen (s.u. Pseudosklerodermie).

Extrakutane UAWs: Fieber (10-25% der Pat.), Hypotonus, Lungenfibrose, Strahlensensibilisierung.

Wechselwirkungen

Cisplatin: kumulative Nephrotoxizität; G-CSF: erhöhte pulmonale Toxizität; Methotrexat: erhöhte Toxizität; Phenytoin: Verminderung des Blutspiegels; Sauerstoff: prolongierte Sauerstoffexposition (Anästhesie) führt zu erhöhter pulmonalen Toxizität.
